This Clarification Text has been prepared by AK 212 Bayrampaşa Diş Sağlığı Hizmetleri Sanayi ve Ticaret Limited Şirketi (“Diş 212” or the “Company”) in accordance with the Personal Data Protection Law No. 6698 (“KVKK”) and relevant legislation, to inform you (the “Relevant Person”) about the collection, processing, and transfer of your personal data.

The Company processes your personal data, which may include financial information, physical space security records, visual and audio recordings, communication data, identity information, professional experience, customer transactions, preferences and needs, personal data, and health data — as a Data Controller within the scope of KVKK.
II. Purpose of Processing Personal Data
Your personal data is processed for the following purposes:

- Execution of employee candidate / intern / student selection and placement processes
- Execution of application processes of working candidates
- Fulfilment of obligations arising from employment contracts and legislation for employees
- Execution of fringe benefits and benefits processes for employees
- Planning and execution of training activities
- Execution of activities in compliance with legislation
- Execution of finance and accounting affairs
- Providing physical space security
- Execution of recruitment processes
- Execution of communication activities
- Planning of human resources processes
- Execution and supervision of business activities
- Execution of occupational health and safety activities
- Receiving and evaluating suggestions for improvement of business processes

III. Data Transfer Purposes and Recipients
Your personal data may be shared with:
- Public institutions and organisations to fulfil legal obligations
- Our suppliers (including legal counsel and accounting firms) to fulfil obligations arising from employment contracts, purchase/sale of goods and services, communication, logistics, finance, and accounting works, and to follow up on legal affairs
- Trusted service providers (such as hotel partners and transfer services) when necessary to fulfil your treatment journey
All third parties with whom we share data are required to maintain the confidentiality and security of your information.
IV. Method of Collecting Personal Data and Legal Basis
Your personal data is processed within the scope of the personal data processing conditions and purposes specified in Articles 5 and 6 of the KVKK. The processing is based on:
- The fulfilment of legal obligations stipulated by law
- The conclusion and performance of contracts
- The legitimate interests of the Data Controller
Personal data may be collected orally, in writing, or electronically, by automatic or non-automatic methods.
Non-automatic methods include phone calls, face-to-face communication, signing contracts, and issuing invoices.
Automatic methods include camera recording systems in common areas and working areas, and email transmission.
V. Rights of the Personal Data Subject Under Article 11 of KVKK
In accordance with Article 11 of the KVKK and the first paragraph of Article 13, you have the following rights regarding your personal data:
- To learn whether your personal data is being processed
- To request information if your personal data has been processed
- To learn the purpose of processing and whether the data is used for that purpose
- To know the third parties to whom your personal data has been transferred, both domestically and abroad
- To request correction of incomplete or inaccurate personal data
- To request deletion or destruction of your personal data within the framework of the conditions stipulated in Article 7 of the KVKK
- To request that corrections, deletions, or destructions be notified to third parties to whom the data has been transferred
- To object to results that arise against you due to the analysis of processed data exclusively through automated systems
- To request compensation for damages incurred due to unlawful processing of your personal data
To exercise these rights, you may submit your requests using the methods below:
a) By personally coming to our company headquarters at Yenidoğan, Abdi İpekçi Cd. No:55, 34030 Bayrampaşa, Istanbul, Turkey
b) In writing to our head office via a notary public or by sending a registered letter with return receipt (to verify identity and ensure information is not given to unauthorised persons)
c) By sending an email from your registered email address to contact@dis212.com, or by other methods determined by the Personal Data Protection Board in the future
Your requests will be concluded as soon as possible considering the nature of the demand, and at the latest within thirty (30) days, free of charge. However, if the action requires extra cost, fees may be charged in accordance with the tariff determined by the Communiqué on the Procedures and Principles of Application to the Data Controller, as set by the Personal Data Protection Board.
