Advanced Periodontal Flap Surgery for Deep Gum Health

When gum disease begins to affect the bone, standard cleaning methods may prove insufficient. At this stage, periodontal flap surgery– lifting the gums to clean the root surgery- is the most effective treatment.

Overview

  • Purpose: To control advanced gum inflammation and eliminate the infection.
  • Logic: By lifting the gum tissue, direct access is provided to previously inaccessible root surfaces and bone defects.
  • Procedure: Under local anesthesia, the gum tissue is opened, the inflammation is cleaned, and the gum is sutured closed.
  • Suitability: patients with advanced periodontitis who have deep periodontal pockets and don’t respond to standard scaling treatment.
  • Recovery: Since it is a sutured procedure, it requires a primary healing period of approximately 7–10 days and careful oral hygiene.
  • Effect: The gum pockets narrow, and the infection is completely eradicated.

Who is Suitable for Periodontal Flap Surgery? Who Should Avoid It?

Periodontal flap surgery in Turkey is preferred in advanced stages where non-surgical methods (curettage and root planing) are insufficient. Individuals with the following conditions require this advanced surgical intervention to prevent tooth loss:

  • Deep periodontal pockets: These are situations where bacteria have penetrated so deeply between the gum and the tooth that they cannot be removed. Pockets at this depth serve as infection foci that the patient cannot clean through brushing.
  • Advanced bone loss: These are cases in which gum disease has not only affected the soft tissue but also damaged the jawbone that supports the tooth. At this stage, surgical exposure of the tissue is necessary to place a bone graft.
  • Recurrent gum abscesses: If inflammation and abscesses persist in the same area despite standard cleanings, this indicates a deep infection focus.
  • Tissue overgrowth (hyperplasia): Conditions where the gums grow excessively due to genetic factors, medication use, or inflammation, making cleaning impossible.

Due to the nature of surgical intervention, this procedure may be considered risky or postponed in certain cases:

  • Uncontrolled medical conditions: Uncontrolled diabetes, severe heart conditions, or immune-suppressing conditions can jeopardize wound healing.
  • Poor oral hygiene habits: Surgical success cannot be achieved in patients who lack the discipline to keep the area clean after the procedure. In such cases, the risk of infection recurrence is very high.
  • Excessive tooth mobility: If more than 70–80% of the supporting bone is lost and the tooth is very loose, extraction may be more appropriate than surgery.

How is Periodontal Flap Surgery Performed?

At Diş 212 clinics, this procedure is performed using a microsurgical technique to completely resolve gum disease. Going beyond a standard dental cleaning, this procedure is carried out in the following stages:

  • Anesthesia and preparation

The area to be operated on is completely numbed using modern anesthesia techniques. This advanced surgical intervention ensures the patient experiences no pain or discomfort. The area is sterilized and prepared for the surgical procedure.

  • Flap incision

Millimeter-precise and controlled incisions are made in the gum tissue. The gum tissue is gently separated from the tooth and the underlying jawbone like a flap. This procedure allows the dentist to directly view root surfaces and bone pockets that would otherwise be hidden.

  • Deep cleaning

This stage is the most critical part of periodontal flap surgery in Turkey. Stubborn tartar on the root surfaces and inflamed tissue are removed using specialized surgical instruments. The tooth roots are made smooth and sterile enough to prevent bacteria from adhering.

  • Final stage

After cleaning and repair, the gum tissue is repositioned at an optimal angle to the root surfaces and bone. Typically, the tissue is adapted slightly above or below its original position to ensure tighter adhesion. Stitches placed with fine surgical sutures seal the area from external factors and initiate healing.

What Happens After Periodontal Flap Surgery?

After the procedure, it takes a little patience for the tissues to reattach firmly to the tooth root and bone. Thanks to the micro-surgical techniques applied by the experts at Diş 212, this process progresses predictably. During recovery, you may experience the following and should keep these points in mind:

  • Immediately after the procedure, the body initiates a natural inflammatory response to protect the surgical site. It is normal to experience mild swelling, tightness, and slight bleeding mixed with saliva in the treated area. Applying intermittent ice packs to the surgical site constricts blood vessels, significantly preventing swelling.
  • During this critical phase, when the sutures are holding the tissue in place, trauma must be avoided. To prevent the sutures from stretching during speaking and chewing, the unaffected side of the mouth should be used. Avoid touching the sutures in the area with your tongue.
  • As recovery after periodontal flap surgery in Turkey, the gums begin to change from a bright red to a normal pink color. Since the root surfaces are cleaned, temporary sensitivity to hot and cold may occur during this period.

Is the Procedure Safe? Risks and Considerations

Since this is a surgical procedure, some temporary, rarely occurring complications may occur. Generally, review the risks associated with periodontal flap surgery:

  • Gum recession: As inflamed and swollen tissues heal and return to a healthy state, the gum line may recede slightly. This actually indicates that the diseased tissue has been removed.
  • Root sensitivity: The cleaned root surfaces become more exposed to external factors. Therefore, temporary sensitivity to hot, cold, or acidic foods may occur.
  • Risk of infection: If post-operative oral hygiene guidelines aren’t followed, there is a risk of infection developing in the treated area.
  • Mild pain and swelling: You may experience mild discomfort in the area after the procedure. This condition typically resolves on its own within a few days.
  • Temporary sensation of tooth mobility: After deep cleaning, the tissues surrounding the teeth enter the healing process. During this process, some patients may experience a mild sensation of tooth mobility.

Post-Treatment Expectations and Results

The primary goal of periodontal flap surgery in Turkey is to protect bone tissue and permanently halt the progression of periodontal disease. Following the flap procedure, the gum pockets significantly narrow. The gums attach more firmly to both the tooth and the underlying bone tissue. This makes it difficult for bacteria to accumulate again, thereby contributing significantly to long-term oral health.

You may experience mild tenderness and discomfort during the first few days of the healing process. However, this is temporary and can be quickly managed with the care routine recommended by your dentist. Within approximately 1–2 weeks, the gums begin to heal and take on a healthier, pink appearance.

Frequently Asked Questions

Periodontal flap surgery is a procedure in which the gum tissue is surgically opened to remove deep infections around the bone.

It is recommended when standard cleaning is ineffective, such as when pockets are deeper than 6 mm or advanced bone loss is present.

Since the procedure is performed under local anesthesia, you will not feel any pain. Although there may be some mild discomfort afterward, it can be easily managed with pain relievers.

The recovery process is usually largely complete within 1 to 2 weeks. However, it may take several weeks for the tissues to fully heal.

It is expected that the gum swelling will subside and the tissues will take on a healthier appearance.

Yes, it completely clears the current infection. However, for the results to be long-lasting, the patient must strictly maintain good oral hygiene.
